Polypharmacy & Safe Prescribing in the Elderly

A two-day simulation-based ADN CoE program focused on safe prescribing and polypharmacy management in older adults. Participants learn essential strategies to prevent medication-related risks.

Training Overview

Polypharmacy & Safe Prescribing in the Elderly is a comprehensive, simulation-enhanced training program designed for geriatricians, internists, clinical pharmacologists, family physicians, emergency clinicians, hospitalists, nurses, and trainees seeking advanced competency in managing polypharmacy, deprescribing strategies, and medication-related risks in older adults. This two-day course integrates structured theoretical modules with realistic prescribing simulations, emphasizing high-risk medication identification, adverse-drug-event (ADE) prevention, and evidence-based therapeutic optimization tailored to frailty and multimorbidity.

The program begins with foundational modules covering geriatric pharmacology, pharmacokinetic–pharmacodynamic changes in aging, multimorbidity interactions, medication cascade mechanisms, and risk stratification for drug-related hospitalizations. Participants explore key principles including high-risk drug classes, cognitive–medication interactions, anticoagulant and psychotropic safety, renal/hepatic dosing adjustments, and validated tools such as STOPP/START, Beers Criteria, and deprescribing algorithms.

Hands-on simulation sessions allow participants to navigate complex prescribing scenarios, identify dangerous drug combinations, manage ADEs, and apply structured deprescribing pathways. Using electronic prescribing simulators and case-based medication charts, attendees practice medication reconciliation, regimen simplification, dose adjustment, shared decision-making, and safety-focused communication techniques for frail or cognitively impaired patients.

Through video-based case reviews, interdisciplinary discussions, and expert-led debriefings, participants analyze real-world polypharmacy failures, ADE root-cause factors, medication-error mechanisms, and system-level prescribing risks. Structured performance feedback enhances therapeutic reasoning, patient-safety awareness, and precision in medication management for older adults.

By the end of the program, attendees will have strengthened their ability to manage complex medication regimens, prevent adverse pharmacologic outcomes, and confidently implement safe, effective, and patient-centered prescribing practices across diverse clinical environments.
