Gastroenterology & Advanced Endoscopy

Specialty Overview

Gastroenterology & Advanced Endoscopy encompasses the diagnosis, medical management and endoscopic treatment of diseases affecting the gastrointestinal tract, liver, biliary system and pancreas.

ADN CoE programmes are designed for physicians seeking structured exposure to contemporary gastroenterology and advanced endoscopic practice. Depending on the selected pathway, participants may observe specialist clinics and endoscopy units, engage in structured case-based learning or undertake a longer-term fellowship focused on longitudinal clinical, endoscopic and academic development.

Programmes may include exposure to diagnostic upper and lower GI endoscopy, gastrointestinal bleeding, inflammatory bowel disease, hepatobiliary and pancreatic disease, therapeutic endoscopy, polypectomy, EMR, ESD, ERCP, EUS and multidisciplinary gastrointestinal care.

Clinical Observership

Observe Contemporary Gastroenterology & Endoscopy Practice

The Gastroenterology & Advanced Endoscopy Clinical Observership is designed for physicians who wish to observe how gastroenterologists evaluate, investigate and manage patients across a broad spectrum of gastrointestinal, hepatobiliary and pancreatic conditions.

The programme is primarily observation-based and focuses on understanding clinical workflows, endoscopy-unit organisation, diagnostic pathways, procedural decision-making and multidisciplinary care.

Clinical Exposure May Include
  • Gastroenterology outpatient consultations

  • Upper GI endoscopy workflows

  • Colonoscopy workflows

  • Gastrointestinal bleeding assessment

  • Polyp detection and characterisation

  • Inflammatory bowel disease management

  • Functional and motility-related disorders

  • Liver disease assessment

  • Biliary and pancreatic disease evaluation

  • Endoscopic biopsy strategies

  • Polypectomy observation

  • EMR and ESD case observation

  • ERCP workflows

  • EUS assessment and procedural planning

  • Gastrointestinal oncology pathways

  • Endoscopy sedation and safety workflows

  • Multidisciplinary case discussions

  • Post-procedural care and follow-up

Educational Format

Participants observe gastroenterology and endoscopy practice under the supervision of experienced faculty and may attend appropriate clinics, endoscopy lists, case discussions, multidisciplinary meetings and departmental educational activities.

The observership does not provide independent patient-care or procedural authority and does not replace local licensing, institutional credentialing or recognised specialist training requirements.

Best Suited For

Physicians seeking:

  • Short-term exposure to contemporary gastroenterology practice

  • Familiarity with diagnostic and therapeutic endoscopy workflows

  • Observation of advanced endoscopic procedures

  • Greater understanding of procedural indications and patient selection

  • International professional development without undertaking a long-term programme

Clinical Immersion

Move Beyond Observation into Structured Endoscopic Learning

The Gastroenterology & Advanced Endoscopy Clinical Immersion Programme provides a more intensive educational experience than a standard observership.

Participants receive structured faculty interaction, guided case review and focused clinical and procedural learning while remaining within the boundaries of local institutional, professional and licensing requirements.

The programme is designed to help participants understand not only what gastroenterologists and advanced endoscopists do, but also how clinical presentation, imaging, lesion morphology, procedural risk and therapeutic objectives influence decision-making.

The Programme May Include
  • Structured review of gastroenterology cases

  • Faculty-guided endoscopy case discussion

  • Diagnostic reasoning in GI disease

  • Review of endoscopic indications

  • Polyp morphology and classification

  • Lesion assessment and optical diagnosis

  • Polypectomy strategy discussions

  • EMR case planning

  • ESD indication and strategy review

  • ERCP patient selection and procedural planning

  • EUS case interpretation

  • GI bleeding management pathways

  • Inflammatory bowel disease case review

  • Biliary and pancreatic disease discussions

  • Complication prevention and management principles

  • Endoscopy quality and safety review

  • Multidisciplinary gastrointestinal case conferences

Educational Emphasis

Clinical immersion focuses on:

Observe → Assess → Analyse → Plan → Discuss → Apply

Participants are encouraged to analyse endoscopic findings, review procedural indications and discuss therapeutic strategies directly with faculty rather than functioning solely as passive observers.

Best Suited For

Physicians who want:

  • More intensive exposure than an observership

  • Structured interaction with gastroenterology and endoscopy faculty

  • Case-based procedural reasoning

  • Greater understanding of lesion assessment and endoscopic strategy

  • Exposure to advanced endoscopy decision-making

  • Focused professional development without committing to a long-term fellowship

Clinical Fellowship

Longitudinal Training in Gastroenterology & Advanced Endoscopy

The Gastroenterology & Advanced Endoscopy Clinical Fellowship is the most comprehensive educational pathway within the specialty.

It is designed for physicians seeking sustained exposure to specialist gastroenterology and advanced endoscopy practice and progressive development in clinical reasoning, lesion assessment, procedural planning, evidence-based management and longitudinal patient care.

Unlike a short observership or clinical immersion programme, the fellowship follows a longitudinal educational model, allowing participants to encounter a broader spectrum of gastrointestinal disease and understand how endoscopic and therapeutic decisions evolve over time.

Fellowship Focus Areas May Include
  • Diagnostic upper GI endoscopy

  • Colonoscopy

  • Gastrointestinal bleeding

  • Polyp detection and characterisation

  • Advanced polypectomy

  • Endoscopic mucosal resection (EMR)

  • Endoscopic submucosal dissection (ESD)

  • Early gastrointestinal neoplasia

  • Barrett’s oesophagus

  • Upper GI lesions

  • Colorectal neoplasia

  • ERCP

  • Biliary obstruction

  • Choledocholithiasis

  • Pancreaticobiliary disease

  • EUS

  • EUS-guided diagnostic procedures

  • Inflammatory bowel disease

  • Hepatology exposure where available

  • Pancreatic disease

  • Gastrointestinal oncology

  • Endoscopic complication management

  • Endoscopy quality indicators

  • Periprocedural patient management

  • Multidisciplinary gastrointestinal care

Fellowship Learning Model

The fellowship may combine:

  • Longitudinal endoscopy-unit exposure

  • Specialist outpatient clinic exposure

  • Faculty-guided case discussions

  • Endoscopic image and video review

  • Lesion morphology assessment

  • Procedural planning exercises

  • Advanced endoscopy case review

  • Multidisciplinary gastrointestinal meetings

  • Academic reading

  • Evidence-based guideline review

  • Case presentations

  • Literature discussion

  • Morbidity and complication review

  • Reflective learning

  • Structured faculty feedback

Where permitted by the host institution and applicable regulations, additional supervised educational activities may be incorporated. The precise scope of participation is determined by the host centre, participant background and local professional requirements.

Learning Objectives

By the end of the fellowship, participants should have developed a stronger ability to:

  • Approach gastrointestinal presentations systematically

  • Understand indications for diagnostic and therapeutic endoscopy

  • Recognise major endoscopic lesion patterns

  • Assess polyp morphology and procedural complexity

  • Understand principles of EMR and ESD case selection

  • Recognise appropriate indications for ERCP and EUS

  • Integrate clinical, imaging and endoscopic findings into decision-making

  • Understand principles of gastrointestinal bleeding management

  • Recognise procedural risk factors and potential complications

  • Discuss evidence-based management of gastrointestinal disease

  • Participate effectively in multidisciplinary gastrointestinal decision-making
